Three deaths resulted from an intentional explosion caused by Charlie Hinder in a caravan containing himself and his two children (ages 4 and 7) during marital separation. Charlie had perpetrated coercive controlling domestic violence against his wife Katherine for years, escalating around separation. Key clinical lessons: recognition that coercive control and psychological abuse without prior physical violence can precede lethal filicide; failure of child safety services to identify specific threats to children; police assessment failure to recognise risk dynamics, treat incidents cumulatively, or conduct formal risk assessment despite visible injury and prior threats; absence of psychological screening for explosives handlers; and systems failure to link domestic violence orders with explosives licensing. The deaths were preventable through earlier intervention, formal risk assessment, and better inter-agency information sharing.
