Julie Anne Holmes, aged 71, died of pneumonia following a cervical spine fracture sustained in a motor vehicle accident that was not diagnosed during her hospital admission. Critical clinical lessons include: (1) paramedics inappropriately removed cervical spine immobilisation despite recognising potential neck injury; (2) the emergency department failed to image her cervical spine despite seatbelt bruising and institutional guidelines requiring radiology for trauma patients over 65 years; (3) a CT shoulder scan failed to detect the cervical fracture; (4) ICU staff did not diagnose the underlying injury despite clinical deterioration. The coroner found treatment was below acceptable standard. Prevention required adherence to existing cervical spine protocols, appropriate imaging in elderly trauma patients, and proper clinical correlation with imaging findings.
