A 20-year-old man with borderline personality disorder, who had made over 25 suicide attempts in the two years prior, died from a gunshot wound sustained during a police encounter. He had called mental health services and Lifeline that morning expressing suicidal ideation, then left his girlfriend's house believing he was getting cigarettes. Police followed and stopped the vehicle he was in; he exited with a large knife and advanced on the officer. The officer, unaware of the knife initially and concerned about threats of a firearm, discharged his weapon in self-defence. The coroner found the shooting legally justified. Critical clinical lessons include: ensuring adequate mental health assessment and follow-up in young people with BPD and multiple suicide attempts; managing acute mental health crisis calls with appropriate emphasis on patient safety and sustained engagement; considering continuity of mental health care following emergency department discharge; recognizing the complex interplay of substance abuse, impulsivity, and mental illness in crisis presentation; and the importance of multi-agency coordination in mental health emergencies.
