Steven William Hazlett, aged 63, died of respiratory failure due to chronic denervation atrophy from axonal degenerative peripheral neuropathy with contributing ischaemic heart disease. He was admitted to hospital with progressive restrictive lung disease and muscle weakness. During his admission, he expressed suicidal ideation, underwent psychiatric assessment, and was managed with appropriate end-of-life planning including a Do Not Resuscitate order. A Mental Health Act order was imposed when he became confused and agitated shortly before death. The coroner found his care and treatment appropriate with attentive monitoring of both physical and mental health. No clinical errors or preventable factors were identified.
