Hock An Ong, 58-year-old Malaysian national, died of acute ischaemic heart disease while evading lawful apprehension during an immigration operation in rural South Australia. He had undiagnosed underlying cardiac disease with cardiomegaly, myocardial fibrosis, and significant left coronary artery atherosclerosis. Mr Ong had a history of hypertension and diabetes, and angina medication was found in his residence, but he had never sought formal cardiac diagnosis. He avoided detection as an unlawful non-citizen for five years despite multiple interactions with government agencies. The coroner found no fault with authorities and determined his death was from natural causes precipitated by stress and exertion during attempted evasion. Clinical lesson: undocumented migrants may delay or avoid healthcare due to fear of detection, resulting in undiagnosed and untreated serious medical conditions.
