Samantha Maslen, a 25-year-old woman with depression and complex social factors, was killed during a police pursuit following an alleged petrol station theft. She was a front-seat passenger when the driver, a fatigued 17-year-old unlicensed driver (Leonard Stanley), attempted to evade police at high speed. Police deployed a tyre deflation device without adequate risk assessment. The driver swerved to avoid the device, causing the vehicle to lose control and roll. Maslen died from cervical spine fracture with spinal cord injury. Critical learning: officers failed to perform proper risk assessment before pursuit initiation; they falsely assumed all occupants were adult licensed drivers; the sudden deployment of the tyre deflation device created a dangerous situation at high speed; and law enforcement objectives were prioritised over safety considerations.
