YGE, a man with major depressive disorder, suicidality, and illicit substance use, died by hanging at the Neighbourhood Justice Centre on 28 December 2019. He was subject to a family violence intervention order and had been arrested for breaches two weeks prior. The Coroners Prevention Unit found no concerns with mental health care provided by St Vincent's Mental Health or family violence support. However, the review identified YGE as part of a vulnerable cohort—people who inject drugs with family violence involvement—who experience earlier death, higher mental illness rates, legal stressors, and reduced service contact despite frequent health service engagement. The coroner recommended that Family Safety Victoria use suicide data from this cohort to inform perpetrator interventions, recognising the intersection of substance use, mental illness, family violence, and suicide risk requires targeted prevention strategies.
