Nicholas Lopes, a 26-year-old with treatment-resistant schizophrenia, intellectual disability, and substance use disorders, died from mixed drug toxicity (heroin combined with benzodiazepines and antipsychotics) two days after absconding from escorted leave at St Vincent's Mental Health inpatient ward. He was a vulnerable patient with multiple previous absconsions typically to use substances. The coroner's Prevention Unit review concluded that granting escorted leave was appropriate clinical management, that no prior indication of absconding intent existed, and that withholding leave would not have prevented his substance use. The treating team acted appropriately in response to the absconding. No preventable clinical errors or missed opportunities were identified. The death highlights challenges in managing patients with concurrent serious mental illness and substance use disorders within involuntary treatment frameworks.
