Adang Akot was a 9-month-old infant with chronic lung disease and subglottic stenosis who died from intercurrent pneumonia and respiratory infection at the Royal Children's Hospital. The coroner was satisfied that the Royal Children's Hospital provided appropriate clinical care throughout her hospitalisation from birth. No protective concerns or medical management issues were identified. The death occurred in the context of pre-existing serious chronic respiratory conditions. The case emphasises that some respiratory infections in children with chronic lung disease and anatomical airway abnormalities can progress to fatal pneumonia despite appropriate medical management. Close monitoring of these high-risk patients and early recognition of clinical deterioration remain important clinical priorities.
