Drew Aaron Boaler, a 39-year-old man with severe intellectual disability, epilepsy, bipolar disorder, and chronic renal failure, died from complications of his renal disease while on an outing. He had presumed lithium-induced renal impairment but was uncooperative with definitive diagnostic procedures like renal ultrasound and biopsy. His condition was further complicated by excessive water drinking requiring environmental restrictions. On the day of death, while being driven by a carer, he developed acute respiratory distress and collapsed. CPR was commenced but unsuccessful. The coroner found no evidence of inadequate care by his residential facility or treating medical practitioners. The clinical lesson is that patients with lithium-induced renal impairment require careful monitoring, dose adjustment, and consideration of alternative mood stabilizers when possible, though discontinuation may cause behavioral deterioration.
