A 72-year-old woman with intellectual disability residing in aged care presented with a large rectal prolapse and ischaemic bowel. She was found on the floor of her room with a prolapse containing approximately 80cm of non-viable small bowel. Despite the poor prognosis even with surgical intervention, appropriate palliative care was provided. The case highlights the challenges of managing complex medical emergencies in patients with severe comorbidities and communication difficulties. Early recognition of the prolapse symptoms at the aged care facility and prompt emergency response were appropriate.
