Finding into death of Mykayla Chloe Marshall

Mykayla Marshall, a 15-month-old child with complex congenital heart disease including total anomalous pulmonary venous drainage, pulmonary vein stenosis, and septal defects, died at home from pulmonary hypertension and right heart failure. She had undergone cardiac surgery within the first day of life and required intensive home management including strict fluid balance, regular medications, daily weighing, and vital sign monitoring. Her carers, Tammy Locke and Glen Smith, provided exceptionally conscientious care. Two days before death, her paediatrician noted worsening heart failure. When Mykayla deteriorated on the day of death, her carer called her cardiac specialist who provided telephone guidance on medication administration. Despite CPR and ambulance resuscitation efforts, Mykayla could not be revived. The coroner found this a natural death with no clinical preventability issues identified.
