A 90-year-old man with extensive cardiac history (previous bypass surgery and pacemaker) died of acute myocardial infarction following pneumonia and cognitive decline. He was admitted to hospital after falls, with documented 10kg weight loss. A myocardial infarction on 13 October was followed by palliative care. Despite chest pain, dyspnoea and troponin elevation on 19 October, he died the next day. The case highlights the importance of recognising end-of-life trajectories in elderly patients with multiple comorbidities and ensuring appropriate symptom management and palliative care escalation in the final stages.
