A 93-year-old man with dementia, heart failure, and chronic renal disease died of general inanition following a hip fracture sustained in a fall at an aged care facility. He was under a guardianship order with special powers. Post-operatively, he developed pneumonia and delirium requiring an inpatient treatment order. After discharge, he had further unwitnessed falls and progressive decline, leading to comfort care. The coroner found the guardianship order lawful, the care at both aged care and hospital appropriate, and made no recommendations. Clinically, this case highlights challenges in managing high-risk falls in frail, cognitively impaired older persons despite preventive measures, and the difficulty balancing autonomy with safety in this population.
