Finding into death of Tai Van Tran

Tai Van Tran, a 43-year-old man with daily heroin use, was arrested and remanded in police custody. He disclosed heroin use during intake assessment and was appropriately scheduled for medical review. On the following day, he developed vomiting consistent with opioid withdrawal and was assessed by a doctor and nurse, both finding standard withdrawal presentation without significant concern. He was prescribed withdrawal medication and appeared stable. That evening, he collapsed in his cell and was found unresponsive approximately 6 minutes later. CPR was immediately commenced and paramedics arrived promptly, but resuscitation was unsuccessful. Autopsy revealed a perforated chronic gastric ulcer, dehydration, and various drugs including methamphetamine and morphine, but the exact cause of death remained unascertained. The coroner found medical care in custody was appropriate, withdrawal management was standard and adequate, and the delay in connecting 000 calls to ESTA ambulance dispatch did not contribute to death given he was already in cardiac arrest.
