A 9-week-old Aboriginal foster child died of sudden infant death (SUID) whilst in approved foster care. The baby was in the Department's care following removal from biological parents due to maternal health issues and parental dysfunction. Post-mortem showed mild respiratory infections (bronchitis, otitis media, metapneumovirus) and evidence the baby was placed prone (face-down) when put to sleep. The coroner identified multiple recognised SUID risk factors including maternal smoking in pregnancy, prone sleeping position, young age, and elevated oxygen requirements from mild respiratory infection. While the foster carer was appropriately trained and provided good care overall, the coroner expressed concern about dilution of the safe sleeping message emphasising back-sleeping position, particularly as emphasis shifted to co-sleeping risks. The precise cause of death was undetermined, but airway compromise from positioning combined with mild respiratory compromise was most likely.
