Finding into death of Dana John Walter

Dana John Walter, 68, died of aspiration pneumonia following cerebral palsy and severe dysphagia. He was readmitted to hospital on 11 August 2020 with acute respiratory distress three days after discharge. Despite thickened fluids and aspiration precautions at his disability accommodation residence, recurrent aspiration pneumonia episodes progressively weakened him. The coroner found care was reasonable and appropriate given his complex needs and recurrent risk. Family concerns about inadequate reassurance during hospital transfer and staffing gaps (exacerbated by COVID-19 agency staff) were acknowledged but not found to constitute deficiencies. The Disability Services Commissioner investigation identified no issues with Home@Scope services.
