GL, a 69-year-old Chinese national, died by hanging in Long Bay Correctional Centre's Medical Services Unit on 12 April 2022. He was incarcerated following the murder of his wife in September 2020 and had become a paraplegic from a subsequent motor vehicle accident. Despite regular mental health reviews from October 2021 onwards—including specialist geriatric and transcultural mental health services—GL consistently denied suicidal ideation while expressing emotional distress about his wife, chronic pain, and physical limitations. He was reviewed daily by nursing staff and received appropriate specialist care. The inquest found his death was intentionally self-inflicted by hanging using bed mechanisms. Clinically, this case highlights the challenge of suicide risk assessment in older prisoners with complex trauma, language barriers, and multiple losses, and the importance of recognising high-risk presentations even when patients deny suicidal intent.
