Finding into death of Janene Louise Weston

Janene Louise Weston, a 50-year-old woman with acquired brain injury from autoimmune encephalitis and history of multiple strokes, died at home in specialist disability accommodation from an intracranial haemorrhage. She was in high-level residential care requiring assistance with most activities of daily living. In 2024, she was treated for presumed deep vein thrombosis with apixaban anticoagulation and underwent surgical management of an infected obstructed kidney. Found unresponsive at home on the morning of 14 October 2024, paramedics declared her deceased at the scene. The coroner's investigation and Coroners Prevention Unit review found the medical care was appropriate. Family concerns about medication changes were reviewed and found unfounded—anticoagulation was medically necessary despite increased bleeding risk. Death was from natural causes and not preventable.
