Sacha Timothee Louis Lefebvre, aged 52, died from complications of chronic cardiac failure with ischaemic heart disease and chronic anaemia whilst sleeping rough at Hawthorn Library. He had multiple chronic conditions including heart failure, unstable angina, type 2 diabetes, and microcytic anaemia, alongside mental health issues and substance use. Despite contact with Boroondara Community Outreach, his homelessness and inability to access stable housing contributed significantly to his premature death. The coroner emphasised that homelessness itself is an independent risk factor for mortality, and that early intervention with housing and wrap-around support (such as the Better Health and Housing Program) could have prevented this death. The case highlights system-level failures in housing policy and healthcare accessibility for vulnerable populations, rather than individual clinical errors.
