Coroner's Finding: YIALAS Dimitrios

A 23-year-old male with drug-induced psychosis died by hanging. He had a long history of substance abuse and behavioural problems, with multiple presentations to hospital and mental health services. In March 2001, he was admitted for acute psychosis but discharged against medical advice. He failed to attend follow-up psychiatric appointments. On 1 May 2001, his mother contacted police expressing serious concerns about his mental state and threatening behaviour. Police attempted to contact drug and alcohol services but received no response. The mental health unit was not contacted. The coroner found the death was possibly preventable through better service coordination and communication systems. Key clinical lessons include: appropriate escalation and documentation of safety concerns; timely psychiatric assessment when acute psychosis is present; accessible crisis intervention pathways in regional areas; and system improvements to ensure concerned family members can reach appropriate mental health services.
