John Joseph Tiernan, 78, died from a large left basal ganglia intracerebral haemorrhage with mass effect while admitted under an Inpatient Treatment Order (ITO) for acute hyperactive delirium. He had stage 4 lung cancer with extensive brain metastases discovered on MRI. His delirium was appropriately attributed to cancer rather than the initial fall. A massive spike in blood pressure precipitated the fatal stroke on 11 May 2017. The coroner found his detention lawful, care appropriate, and no concerns were raised by family or investigators. This case illustrates the complexity of managing delirium in advanced cancer patients and the importance of thorough investigation (MRI) to exclude structural causes before attributing confusion solely to psychiatric factors.
