A 65-year-old woman with intellectual disability, congenital blindness, diabetes, and epilepsy presented with an incarcerated umbilical hernia requiring emergency surgery. Post-operative aspiration occurred in recovery with subsequent aspiration pneumonia. Despite appropriate management including ICU consultation, the patient's severe anxiety, agitation, pain, and comorbidities led to clinical deterioration. The coroner found the healthcare provided was appropriate and consistent with Victorian public health standards. No clinical management failures or want of care were identified. The death resulted from aspiration pneumonia as a direct complication of emergency surgery, not from preventable medical errors.
