A 90-year-old woman with multiple comorbidities residing in aged care sustained a hip fracture from a fall in January 2018. Following surgical repair and initial hospital treatment, she was transferred to a rehabilitation facility (Peter James Centre) where she suffered a second fall resulting in additional fractures. She was readmitted to hospital, developed hospital-acquired pneumonia, and died in February 2018. The coroner found care was reasonable and appropriate, though noted systemic issues regarding staff ratios in managing simultaneously competing urgent demands of multiple patients, particularly elderly patients with cognitive and physical decline who remain mobilising. The family's concerns were largely directed at systemic resource constraints rather than individual clinician failures.
