A 29-year-old man with a history of heroin and methamphetamine addiction died from mixed drug toxicity. He had suffered a work-related back injury leading to opioid dependence, complicated by anxiety and panic attacks. Although engaged with psychological and drug and alcohol counselling services, and maintained on methadone with good medication compliance, he died from an unintentional overdose at home. The coroner identified systemic issues with addiction services, particularly during COVID-related restrictions, and highlighted the oversupply of prescription opioids and benzodiazepines as contributing to addiction burden. The case underscores the need for improved multidisciplinary addiction management, better access to detoxification services, and careful prescribing practices to prevent iatrogenic addiction.
