A 14-year-old boy from Scotland died of severe heatstroke with myocarditis while hiking in extreme heat (40°C+) at Cape Range National Park. He had recently recovered from a viral illness. His father made navigation errors that prolonged their exposure. Police failed to immediately dispatch an ambulance when the father called at 2:03pm reporting a collapsed child with heat exhaustion—the critical gap. Although paramedics arrived by 3:30pm, the boy was already in cardiac arrest after three hours of extreme heat exposure and lying on rocks heated to 52-57°C. Emergency personnel provided extraordinary rescue efforts but could not save him. Key clinical lessons: (1) recognize heat-related illness urgently in vulnerable populations; (2) 000 call takers must immediately dispatch ambulances for collapsed persons, not delay on logistics; (3) acclimatization is inadequate protection in extreme heat; (4) myocarditis increases heat vulnerability; (5) rapid cooling and early hospital access are critical—each minute matters.
