Eileen Sarah Elliott, aged 77, died of myocardial infarction following a fall and leg fracture in January 2000. She was admitted to Glenside Hospital under a Mental Health Act detention order for chronic schizophrenia. After external fixation at Royal Adelaide Hospital, her general condition deteriorated significantly. She suffered a cardiac arrest on 3 March 2000 and was diagnosed with anterior myocardial infarction. Management focused on comfort measures rather than invasive investigation, in discussion with family. She died on 14 March 2000. The case highlights the importance of recognising death in custody triggers for coronial investigation, and ensuring appropriate cardiac monitoring and assessment in vulnerable, detained patients with acute cardiac events.
