This coronial inquiry examined a shooting incident on 3 August 1999 where Rodney Ansell, a man suffering from deteriorating mental health and chronic amphetamine abuse, opened fire on the Stuart Highway killing Sergeant Glen Huitson and wounding three civilians. Ansell had developed a shared delusional system with his de facto partner involving beliefs about Freemasons kidnapping his children. His amphetamine use and possible underlying mental illness created a paranoid psychotic state. Sergeant Huitson was fatally wounded by a bullet that penetrated his ballistic vest through the unfastened side flaps; a properly secured vest may not have prevented injury but proper fastening protocols were inadequately taught. The incident revealed police training and equipment deficiencies in roadblock protocols, portable radio communications, and ballistic vest instruction, though these did not contribute to the deaths. Constable O'Brien's exceptional tactical response and courage prevented further casualties.
