Arthur Andrianakis, 51, died from hyperthermia of unknown cause with cardiac hypertrophy and COPD. He presented to hospital with respiratory distress following police custody for assaulting his mother. Initial diagnosis was COPD exacerbation with infection, complicated by pulmonary embolism. During ICU admission he developed unexplained severe hyperthermia (43°C) despite appropriate treatment including active cooling and dantrolene. The coroner found the hospital care reasonable and appropriate. Key clinical lessons: the underlying cause of hyperthermia remained unclear despite extensive investigation; cardiac hypertrophy with infection and physiological stress created high-risk conditions for deterioration; early recognition of his complex medical and psychiatric history may have informed management, though the coroner did not identify preventable care failures.
