A 5-week-old infant died from fatal injuries inflicted by the family pet, a dog (American Staffordshire Terrier) named Bully, who attacked him in the family home in early July 2021. About one month before the fatal attack, Bully had attacked and killed a neighbour's dog, which prompted the council to issue a notice of intention to declare him a restricted dog under the Companion Animals Act. However, the declaration process had not been completed by the time of the fatal attack. The coroner identified several systemic failures: misunderstanding of interim seizure powers by council officers, ambiguous legislation regarding seizure authority, lack of muzzling requirements during the interim period pending declaration, absence of owner licensing requirements, and difficulties in accurately identifying dog breeds (the council initially misidentified Bully as a pit bull cross when he was actually an American Staffordshire Terrier). The coroner made ten recommendations focused on strengthening the regulatory framework, including legislative amendments, owner licensing, public education campaigns, enhanced penalties, and clearer seizure powers.
