Mrs Chrupalo, a 73-year-old woman with physical disability, cognitive impairment and alcohol dependency, was discharged from Dubbo Base Hospital on 8 July 2015 against medical advice despite being bedbound and dependent on her son for total care in squalid living conditions. A psychiatrist's rushed capacity assessment did not adequately evaluate her cognition or family circumstances. The Hospital failed to make a guardianship application despite clear indicators of neglect and elder abuse. Lourdes community nursing discharged her in September 2015 based on representation by her son rather than direct communication with Mrs Chrupalo. When the Public Guardian was appointed in October 2015, critical follow-up steps were not taken: no attempts were made to contact alternative service providers, verify information from the applicant, or request police welfare checks despite clear signs of crisis. Mrs Chrupalo was found deceased approximately three months later in poor condition. Clinical lessons include: recognizing incapacity based on physical disability not just cognitive ability; direct communication with vulnerable patients avoiding reliance on carers as gatekeepers; timely discharge planning with confirmed service availability; and escalation to protective authorities when concerning circumstances persist despite initial interventions.
