A 56-year-old man with congenital microcephaly, intellectual disability, dysphagia and recurrent aspiration pneumonia died from aspiration pneumonia whilst receiving palliative care in hospital. He had multiple episodes of aspiration pneumonia in the months preceding death. The coroner found the care provided was appropriate, including the clinical decision to transition to palliative management given his underlying congenital disability and recurrent infections despite treatment. The case highlights the challenges of managing aspiration risk in patients with severe dysphagia and congenital disability, though no specific clinical failings were identified. The death was from natural causes.
