A 52-year-old man with a 25-year history of depression, anxiety, and severe obsessive-compulsive disorder (OCD) died by hanging. He had recent contact with mental health services including a psychiatrist, psychologist specialising in OCD, and a hospital crisis triage service. While he denied suicidal ideation on formal assessment in May 2019, he expressed increasing suicidal thoughts during a June crisis call but declined admission. The coroner made no criticism of treating clinicians but recommended that professional colleges (RACGP, RANZCP, APS) review guidance on OCD treatment, emphasising the utility of involving family members and gathering collateral information, particularly when obsessive thinking may carry self-harm risk. The case highlights the tension between patient confidentiality and safety in severe mental illness, and the importance of family engagement in treatment planning.
