Finding into death of John Threadgold

Mr John Threadgold, a 55-year-old man with extensive psychiatric history (schizoaffective disorder, substance dependency), chronic obstructive airways disease, and alcohol abuse, died from cardiopulmonary arrest and respiratory failure. He was admitted involuntarily under the Mental Health Act in July 2010 and transferred to a Prevention and Recovery Care unit. His condition deteriorated with worsening breathlessness exacerbated by heavy smoking. He was readmitted to the ED on 6 August 2010 with acute myocardial infarction and COAD exacerbation, deemed not for resuscitation due to poor prognosis, and died the following day. A critical clinical lesson: the death was not reported to the coroner at the time despite the deceased being a person in care, no forensic pathology examination was performed, and integrated medical-psychiatric management of patients with concurrent severe mental illness and chronic physical disease requires careful coordination to prevent deterioration.
