Coroner's Finding: SMITH Colin Peter

Colin Peter Smith, aged 51, died of respiratory and cardiac failure from acute pulmonary oedema and pneumonia on a background of chronic obstructive lung disease and ischaemic heart disease. He presented to the Royal Adelaide Hospital acutely short of breath and confused. Dr R. appropriately initiated non-invasive ventilation and diuretics, diagnosed acute pulmonary oedema and pneumonia, and issued a Mental Health Act order. When Mr Smith failed to respond to treatment, the treating team, including ICU registrar and ED consultant, appropriately determined that invasive airway management was not indicated given his poor prognosis and would offer minimal benefit. Palliative care was then provided. The coroner found the detention lawful, the medical assessment and management appropriate, and made no recommendations. This case demonstrates appropriate clinical decision-making in a complex patient with multiple comorbidities and acute decompensation.
