Finding into death of Anastasios Kosmas

Anastasios Kosmas, 60-year-old male with psychiatric history (manic schizophrenia, depression) and cardiovascular risk factors (diabetes, hypertension, hyperlipidaemia), collapsed on 19 May 2013 at a secure psychiatric extended care unit. Imaging revealed large left middle cerebral artery stroke affecting two-thirds of the hemisphere. A multidisciplinary team (ICU, stroke, psychiatry, social work) assessed him as unsuitable for surgical intervention with poor prognosis. Following family discussion, comfort care was provided with extubation on 23 May 2013, and he died on 24 May 2013. The coroner found the death was due to natural causes (ischaemic cerebrovascular accident) requiring no further investigation.
