Inquest into the death of Samuel SEETO

Samuel Seeto, a previously healthy 29-year-old, died from florid myocarditis after a general practitioner failed to detect his cardiac condition. He presented on 5 December 2015 with a 3-day history of fatigue, feeling cold, and multiple episodes of syncope including collapse in the waiting room. Dr P. diagnosed viral illness with dehydration and discharged him with advice to take fluids and ibuprofen. Autopsy revealed severe myocarditis with pleural effusion. Expert evidence established that appropriate investigation with ECG and chest X-ray would likely have detected the condition and led to hospital admission. Key clinical lessons: syncope is concerning and mandates cardiac evaluation; young patients with fatigue, low blood pressure, and syncope require careful differential diagnosis including cardiac causes; chest auscultation should be performed properly; available investigations (ECG, X-ray) should be utilised when clinically indicated.
