Christopher O'Sullivan died aged 29 days from head injury of undetermined aetiology. He presented on 15 September 1993 with fixed pupils, cyanosis, and seizure-like activity, progressing to cardiac arrest. Autopsy revealed subdural, subarachnoid, and retinal haemorrhages. Expert pathologists reviewed the case in 2009-2010 and concluded non-accidental blunt force injury was most likely. However, without evidence of violence, clear mechanism, or witness testimony, the coroner found the standard of proof insufficient despite serious allegations. The coroner's cautious approach reflected the Briginshaw principle: serious allegations require strong evidence. Key lessons include careful interpretation of medical findings in infants with head injury; the need for early, thorough investigation when suspicious injuries are identified; and consideration of alternative natural causes including birth trauma, resuscitation effects, and slowly-developing encephalopathy. Improved early suspicion and comprehensive investigation might have clarified causation.
