Shayne McMahon, 20, died on 7 April 2007 from cardiac arrhythmia and hypoxic brain injury after exposure to butane from a severed LPG gas line at North Star Holiday Resort. As an unlicensed apprentice plumber/gasfitter, he was sent unsupervised to break into a live gas line in a ring main system—a complex task requiring licensed tradesperson supervision. Critical safety failures included: no supervision by his licensed employer, incomplete training (prior experience limited to dead-end lines requiring one clamp, not ring mains requiring two), no Job Safety Analysis completed, inadequate safety equipment, and no gas network map. Expert toxicologists concluded butane-induced sudden cardiac arrhythmia caused collapse. The death was entirely preventable through proper supervision, comprehensive training, and adherence to safety procedures. The coroner made far-reaching recommendations regarding industry safety alerts, improved Material Safety Data Sheets, TAFE training reforms, and regulation of private gas distribution networks.
