Coroner's Finding: De-identified PT

PT was a 14-year-old boy with a history of anorexia nervosa and bulimia who died by hanging in April 2022. He had been discharged from eating disorder treatment in November 2021 after maintaining appropriate weight. In the months before his death, family and friends observed he appeared to be thriving. The night before his death, he posted an inappropriate selfie on Snapchat that attracted negative comments. The next morning he expressed reluctance to attend school but denied feeling unsafe. Hours later he was found hanging. While the coroner identified a failure by Strong Families Safe Kids to formally refer an unrelated June 2021 sexual allegation to police, this administrative oversight had no temporal or causal connection to his death. The coroner found no suspicious circumstances and concluded PT's actions were voluntary with intent to end his life.
