A 3-year-old girl was killed by her mother, who was experiencing a severe psychotic episode with suicidal and homicidal ideation. The mother had a history of Brief Psychotic Disorder (diagnosed 2015), was on olanzapine 15mg prescribed by a psychiatrist, but had a pattern of medication non-compliance (adjusting doses without medical advice). Key clinical lessons: the referring psychiatrist's management plan to the GP lacked detail regarding warning signs, crisis management, medication compliance strategies, and follow-up frequency; responsibility for ongoing care was unclear between psychiatrist and GP; the mother remained socially isolated with poor English proficiency and no driver's licence, creating barriers to help-seeking; and GPs need more assertive follow-up systems for high-risk mental health patients who disengage. Better coordination between psychiatry and primary care, comprehensive management plans addressing psychosocial factors, and improved medication compliance monitoring could potentially have prevented this tragedy.
