Frank Burrows, a 58-year-old male prisoner, died from Haemophagocytic Lymphohistiocytosis (HLH) secondary to T-cell lymphoma. He presented with non-specific symptoms (fever, sweats, diarrhoea) and was initially discharged from hospital on 10 November 2016, then readmitted 2 days later. Diagnosis of HLH was made on 16 November, but despite ICU management, he deteriorated rapidly into multi-organ failure and died 7 days later. A forensic medical review found that while earlier haematology review might have diagnosed HLH marginally sooner, the outcome would not have changed given the rarity of the condition, rapid progression, and the 11-day interval between admission and death. Prison medical care was thorough and hospital care was comprehensive and appropriate. No preventable factors were identified.
