A 51-year-old woman died from mixed drug toxicity (amitriptyline, chlorpheniramine, diazepam, codeine) complicated by biventricular heart hypertrophy. She had chronic back pain treated with opioids and used excessive over-the-counter antihistamines for perceived allergies and their sedating effects. Despite counselling from family and her GP to reduce medications, she continued accumulating and consuming multiple CNS depressants. Six days before death, police found her difficult to rouse; she declined hospital assessment. The coroner found no evidence of intentional self-harm. Clinical lessons include recognising medication accumulation risks in socially isolated patients, the dangers of polypharmacy with CNS depressants particularly in those with cardiac pathology, and the importance of structured follow-up when patients decline assessment after concerning presentations.
