Luke Trevor Young, 42, died from mixed prescription drug toxicity (sertraline, amlodipine, lamotrigine, quetiapine) in a suicide by overdose. He had severe, longstanding bipolar II disorder with chronic suicidality, complicated by alcohol/drug dependence and multiple physical comorbidities. His GP provided comprehensive care with 50 consultations over 11 months and appropriate psychiatric referrals. However, the coroner identified a critical gap: lack of intensive community-based mental health case management in the North West region. Dr K. (psychiatrist) confirmed that ideal care would involve weekly/fortnightly multidisciplinary team coordination, which was unavailable. When Mr Young later sought another psychiatrist, he received only brief teleconference consultations (under 6 minutes each). The coroner commended his GP but noted that intensive local case management may have assisted despite poor prognosis.
