A 91-year-old woman died from pulmonary thromboembolism and abscess formation complicating untreated ankle fracture and pressure sores. She had sustained a fracture-dislocation of her right ankle after a fall, which she refused medical treatment for, resulting in permanent deformity and immobility. Pressure sores developed over weeks. Her daughter, a nurse, left her alone for two weeks despite recognising mobility risks and her mother's frailty. The deceased was mentally alert and strongly refused medical care, and insisted her daughter proceed with the overseas trip. Dr G. stated the death was preventable had the ankle fracture and pressure sores received appropriate medical treatment. This case highlights the tension between respecting elderly patient autonomy and duty of care, and the serious risks of immobility, untreated fractures, and pressure ulcers in elderly populations.
