Inquest into the Death of Dean Anthony POLLARD

Dean Anthony Pollard was a 41-year-old man with a long history of schizophrenia and amphetamine-induced psychotic disorder who disappeared in May-June 2010 and is presumed dead. He had multiple psychiatric admissions between 2002-2010 with diagnoses of paranoid schizophrenia, schizo-affective disorder, and amphetamine-induced psychotic disorder. His last recorded contact was 17 May 2010 at Swan District Hospital where psychiatric assessment found no acute risks despite chronic substance use and aggression. He had poor medication compliance, escalating drug dependence, and episodes of violence. Clinical lessons include: psychiatric patients with dual diagnosis (mental illness plus substance abuse) require sustained engagement; single assessments showing stability may miss chronic risk; the biopsychosocial trajectory toward withdrawal from services is common and dangerous; and structured follow-up with crisis planning is critical in this population.
