A 55-year-old man with alcohol dependency died from combined alcohol and promethazine toxicity with heat exposure. He spent 4-5 hours sunbaking in high temperatures (exceeding 30°C) while heavily intoxicated and sedated by antihistamine medication. His body temperature reached 45°C. The sedating effects of alcohol and promethazine prevented him from recognising danger or moving to safety. He had presented to hospital 10 days earlier following an intoxication-related fall, where he was counselled on alcohol withdrawal but was unable to reduce consumption. The coroner noted that excessive alcohol consumption causes respiratory, cerebral and cardiac depression that prevents protective responses to environmental threats. Clinical lessons include recognising that patients with severe alcohol dependency remain at high risk despite brief interventions, and understanding how CNS depressants impair thermoregulation and protective reflexes.
