Ian John Davidson, aged 60, died from a self-inflicted stab wound to the chest while police attended his partner's home to enforce bail conditions requiring him to stay away from her. Six days earlier, psychiatric assessment at Royal Adelaide Hospital found no acute psychiatric diagnosis or self-harm risk. A consultant psychiatrist's earlier report noted recurrent depressive disorder with dependent personality features and risk of impulsive self-directed actions. Police acted appropriately and with restraint, only entering the premises when concerned for his safety. By the time they located him, resuscitation was unsuccessful. The coroner found police bore no responsibility and made no recommendations.
