Coroner's Finding: Maynard, Grant Godfrey

60y · Male·multi-organ failure due to injuries sustained in motor vehicle accident, specifically fractured ribs and collapsed left lung with haemothorax

A 60-year-old Aboriginal man with significant comorbidities (cardiomyopathy, cirrhosis, renal failure, diabetes) presented to ED following a motor vehicle accident at low speed. Despite chest imaging showing pleural effusion and the patient having rib pain, fractured ribs were not diagnosed. Subsequently, a haemothorax developed with associated bleeding, circulatory compromise, and acute kidney injury. The medical advisor found care was below standard, specifically noting failure to diagnose rib fractures and failure to recognise significant bleeding. The patient deteriorated despite late intervention and died from multi-organ failure. Earlier recognition of the injury pattern and bleeding complications might have enabled more timely intervention.
