Inquest into the deaths of Kayla EWIN and Iziah O'SULLIVAN (Annexure to finding)

This document is not a coroner's finding about a specific deceased person. It is a SUDI (Sudden Unexpected Death in Infancy) classification framework used by NSW coroners to categorise unexpected infant deaths. The classifications range from SUDI 0 (insufficient post-death investigation) to SUDI 3 (unsafe sleeping environment with modifiable risk factors), plus categories for explained deaths and undetermined cases. Each classification defines required investigative standards and identifies intrinsic (non-modifiable) and extrinsic (modifiable) risk factors such as sleep position, bed-sharing, bedding compliance, tobacco exposure, and appropriate dressing. The framework guides systematic assessment of infant deaths to determine cause and preventability.
